📢 新版保險金申請書啟用公告

本校學生團體保險承保之保險公司（遠雄人壽）自 115年01月01日 起修訂保險金申請通知書。請配合自民國115年01月01日起使用新版「保險金申請書11501版」（共有3頁，詳附件電子檔）。如欲申請學生團體保險金，即日起敬請配合使用新版之保險金申請書，謝謝。

Starting Jan 1, 2026, Farglory Life Insurance has updated the claim application form. Please use the new "Claim Application Form 11501 Edition" (3 pages) for all future claims.

理賠申請應檢附資料 Required Documents

理賠申請書
Claim Application Form.
診斷證明書：
請於最後就診日申請，並請醫師註記全部看診日期。
Medical Certificate (Apply on your last visit and ask the doctor to note all consultation dates).
收據明細：
若有個人保險需申請，繳交加蓋院所章之副本即可。
Receipt Details (If you need originals for personal insurance, copies stamped by the hospital/clinic are acceptable).
存摺封面影本：
理賠金額匯款確認用。
Copy of Passbook Cover (For remittance confirmation).
平安保險費繳費證明：
請自行登入校務系統列印。
Proof of Premium Payment (Please log in to the school system to print).
【特定事故】骨折：請檢附骨折部位光碟片。
若需退還請事先告知，退還費用由保險理賠金中扣除。(Please provide an X-ray CD. If you need it returned, notify in advance; the return fee will be deducted from the claim.)
【特定事故】交通事故：若有相關證明文件，請一併檢附。
Traffic Accidents: Please attach relevant proof documents if available.
【特殊身分】未成年者：
請法定代理人簽名並蓋章，並檢附身分證正反面影本及戶口名簿影本等關係證明文件。
Minors: Requires legal guardian's signature/stamp, along with copies of ID (both sides) and household registration.
【特殊身分】港、澳及外籍生：
請檢附居留證正反面影本。
Foreign, HK, and Macao Students: Please attach a copy of the ARC (both sides).

特殊票據變更處理 Special Check Processing

★ 自 113年10月1日 起，更新【付款票據變更申請書1131001版】。
★ 若您的帳戶為警示帳戶必須開立支票，需填寫「新版付款票據變更申請書1131001版」勾選並由本人簽名，辦理下列選項（二擇一）：

「取消平行線」：受款人可親臨發票銀行領取現金。
Cancel parallel lines: Payee can receive cash in person at the issuing bank.
「取消禁止背書轉讓」：辦理完畢後，可以將支票存入他人帳戶。
Cancel prohibition of endorsement transfer: The check can be deposited into someone else's account.
